At Breaking Barriers Therapeutic Center, we offer services that support healing, growth, and meaningful change for children, teens, and adults. Using thorough assessments, evidence-based approaches, and personalized treatment plans, our caring staff work with individuals facing stress, trauma, emotional difficulties, behavioral concerns, and major life changes.

Intake Assessment Information

Before starting any office-based services, an intake assessment must be completed. This first appointment gives our clinical team the opportunity to learn more about your needs and determine the best way to support you.

Please review the following details before your appointment:

  • Intake appointments usually take about 30 to 45 minutes.

  • The intake fee is $40, unless another rate has been arranged.

  • Payment can be made with cash (exact change only) or credit/debit card.

  • Please bring a current photo ID and proof of income, if needed.

  • If you have Medicaid or another insurance plan, please bring your insurance card.

  • If your referral comes from the court system, please bring any related court paperwork so we can place you in the correct service.

  • Children younger than 13 may not stay in the waiting area without supervision. Please plan accordingly.

Information for Parents and Guardians

For children and adolescents receiving services:

  • The first intake appointment is for the parent or legal guardian only. Unless your clinician tells you otherwise, please do not bring the child to this visit.

  • A parent or legal guardian must attend the intake to give permission for treatment before services can begin.

  • Biological parents should bring a valid photo ID.

  • Non-biological legal guardians must bring a valid photo ID along with official documents showing guardianship.

  • We cannot begin services for a minor without written consent from a parent or legal guardian.
